4 + 4 medical education: a word of caution
We read with interest the Comment by Chen Wang and colleagues,' which introduced the new 4+4 medical education programme launched in Peking Union Medical College in September, 2018. However, caution must be taken in describing this programme as a success, because medical students have yet to graduate.
A 5-year undergraduate medical programme is common in China and has also achieved high standards of education for doctors in Europe and Australia, highlighting the potential for adopting pathways other than theUS 4 +4 model. Student experiences from these countries should be carefully studied to improve the quality of medical education in China, rather than developing a new programme without adequate infrastructural support.
The US 4 + 4 model was adopted in 2005 by Zhejiang University, but with little improvement in the quality of education. Medical education should not be designed to support leading medical talents, but rather to qualify medical doctors. Given the stress and challenges that Chinese doctors experience,' the new 4+4 model is unlikely to recruit excellent college graduates. Institutional leadership within medical education should convince the Chinese Government to acknowledge the difference between professional and academic degrees, thereby rewarding medical students for their academic achievements and continued learning. Moreover, medical students are expected to spend their last year as interns, which might further compromise the quality of education by shortening the study duration of basic and clinical medicine.
